The Community Health Assist Scheme (CHAS) is a national initiative by the Singapore government that enables Singapore Citizens to receive subsidised healthcare at participating GPS and private medical and dental* clinics.
*Only for CHAS Blue/Orange, Pioneer Generation, and Merdeka Generation cardholders.

CHAS Eligibility Criteria
All Singaporean citizens are eligible for CHAS. Subsidy levels depend on household monthly income per person or the Annual Value (AV) of your home:

CHAS Blue, Orange, and Green each offer different subsidy levels:
- CHAS Blue/Orange – Provide lower-income households with higher subsidies for treating common illnesses, chronic conditions and dental services.
- CHAS Green – Allows all Singaporean citizens with certain chronic conditions to receive subsidies for treatment costs, regardless of their income.

CHAS & CDMP (Chronic Disease Management Programme)
Chronic diseases are long-term health conditions that could worsen over time and typically require ongoing medical attention. Under CDMP, you may receive subsidised care for 23 common chronic diseases at CHAS-accredited clinics.

How to Apply for CHAS / Renew Your Card
Applying for CHAS is straightforward. If you already have a CHAS card, remember to renew it before expiry to continue enjoying subsidies.
How to apply or renew:
- Visit the CHAS application portal.
- Log in with SingPass.
- Submit your application and supporting documents.
- Receive your CHAS card via mail within a few weeks.
Alternatively, you may complete and sign a hardcopy form and mail it to Bukit Merah Central Post Office, P.O. Box 680, Singapore 911536.
