Why Is It Important To Enrol In Healthier Sg?
Chronic diseases become more rampant as people age and can greatly affect each person’s quality of life as well as their families. Long-term medical conditions do not only take a toll on the body’s physical health, but also the mental and emotional health. Furthermore, the cost of getting treatments greatly impacts one’s finances, which can be onerous for families.
To help alleviate the burdens that chronic diseases bring, Healthier SG enrolment provides eligible Singaporeans with a healthcare programme that empowers them to build a healthier lifestyle and avoid diseases.
Who Is Eligible To Enrol In The Healthier Sg Programme?
Citizens and Permanent Residents of Singapore aged 40 and above are eligible to enrol in the Healthier SG enrolment programme. The enrolment prioritises those who have been diagnosed with a chronic illness or those who may exhibit conditions that need medical care. Singaporeans who pass the criteria for eligibility may enrol through the HealthHub app or in one of the enrolment stations nearest them.

Can I Make Appointments At Healthier Sg Clinics?
There are select activities and programmes that are accommodated for walk in, although availability is limited. If you would like to get a guaranteed slot, you have to enrol via the HealthHub app to book an appointment.
In case you don’t have access to the HealthHub app, you may visit an Enrollment Station and seek help from one of the Healthier SG ambassadors to set-up your appointment. An appointment card will be given for you to bring during your appointment.
What Is Included In The Healthier Sg Health Plan?
The Healthier SG Health Plan is designed for each enrollee to manage their health and wellness by taking action on the various strategies to achieve their health goals. These goals are formulated together with a certified Healthier SG doctor and community partners.
A Health Plan is used as guidance for both enrollees and doctors in order to:
- Allow enrollees to have a better understanding of their health’s general state
- Enable doctors to ensure the correct and timely provision of care to prevent disease and manage chronic diseases
- Develop health and lifestyle goals that involve the cooperation between enrollees and doctors

- Health Log (Height/Weight/Bmi/Vital Signs)
- Health Screening Programme Guide (Screen For Life)
- Target And Action Plans To Achieve Health Goals
- Tracking Of Immunisations

You will receive a Health Plan Booklet during your initial appointment with your chosen doctor. Each health plan is different depending on your health needs and goals, which are discussed during your first Health Plan consultation.
What Types Of Healthcare Subsidies Are Available From The Government In Singapore?
The following healthcare subsidies from the Singapore government are available for use under Healthier SG:
- Community Health Assist Scheme (Chas)
- Medisave
- Merdeka Generation
- Pioneer Generation
- Subsidies for recommended national health screenings and vaccinations (limited to Singapore Citizens)
Through these government subsidies for healthcare, Healthier SG enrollees can avail these benefits:
- One-time free initial Health Plan with the chosen clinic
- S$20 worth of vouchers (3,000 Healthpoints) rewarded following the initial Health Plan consultation via Healthy 365 app
- Fully-subsidised for recommended national health screenings and vaccinations (limited to Singapore citizens)
- CHAS cardholders can use the Healthier SG Chronic Tier to pay for treatments with provision of up to 87.5% in subsidies on chronic medications that are whitelisted.

Are There Any Restrictions On The Use Of Healthcare Subsidies?
Healthcare subsidies under Healthier SG are only applicable to eligible enrollees to the programme. To qualify, you must be a Singapore citizen or permanent resident who is 40 years old or older.
Additionally, the subsidy for chronic conditions medications are restricted under the Healthier SG Chronic Tier.
Is There A Difference Between Healthier Sg General Practitioner (Gp) And Polyclinics?
Healthier SG uses a standard care protocol for all accredited GP and polyclinics. This is to ensure that enrollees receive uniform care from doctors and healthcare providers in managing health plans and chronic diseases. Similar to polyclinics, each Healthier SG GP clinic is supervised by a Primary Care Network and supported by a team of healthcare professionals.
When it comes to consulting with doctors, it is not a guarantee that you will always see the same doctor each time you visit a polyclinic. This is because polyclinics work in teams and may have several doctors to be able to cover the demand of appointments. Despite this arrangement, you can trust that the consistency and quality of care provided by the team are the same. Your health information will be updated accordingly and shared within the team for effective continuity of care.
What If My Existing Gp Is Not Under Healthier Sg?
You will not be covered by any of the Healthier SG benefits if your existing GP is not under the programme, but you can still avail of the subsidies under CHAS and MediSave. If you would like to avail of all Healthier SG benefits, choose a Healthier SG GP or polyclinic nearest you.
What Are Healthpoints?
Healthpoints are incentives given when you complete healthy lifestyle challenges and programmes run by the Health 365 app. These points are convertible to:

It is important that you have a Healthy 365 account as all Healthpoints will be rewarded in the app. You can track the expiration of your Healthpoints in the Rewards section under “My Wallet” in the Healthy 365 app.
